Signature HealthCARE is hiring Licensed Practical Nurses (LPNs) to join our Hermitage, Hillcrest, and Riverside locations. You will provide compassionate nursing care, support residents and families, and collaborate with the healthcare team to deliver quality care.

The role requires an active LPN/LVN license in the state of practice and may include supervisory duties. CPR/BLS certification is preferred, with at least one year of clinical nursing experience, preferably in long-term care.

Competenze

Current, active LPN/LVN license in the state of practice.
CPR/BLS certification preferred (American Heart Association or American Red Cross).
Minimum one year of clinical nursing experience; long-term care experience preferred.
Experience in a supervisory or charge nurse role, preferred.
Familiarity with electronic health records (EHR) systems.
Proficiency with Microsoft Office applications.

Mansioni

Provide direct nursing care to residents, including medication administration and wound care.
Assist residents with ADLs and ensure dignity, comfort, and safety.
Supervise nursing assistants and medication aides during the shift per policy.
Monitor residents and report changes to physician.
Develop and review individualized resident care plans with the team.
Complete clinical documentation per facility policy.
