Job summary

Valir PACE is seeking a Licensed Practical Nurse to provide in-home, center, and clinic nursing care under the participant's Plan of Care. You will work with the RN Care Coordinator and IDT to ensure safe, coordinated, participant-centered care, including on-call coverage and documentation.

The role involves clinical visits, medication administration, education for participants and families, and collaboration across nursing and home care teams to maintain continuity of care.

Qualifications

• Current Oklahoma LPN License without restrictions.
• One year of experience with frail and elderly population or approved training.
• Current CPR certification.

Responsibilities

• Provide nursing visits per Plan of Care in home, center, and clinic.
• Administer medications and treatments as ordered; educate participant and caregiver.
• Collaborate with RN Care Coordinator and interdisciplinary team for coordinated care.
• Complete accurate and timely documentation of visits and communications.
