Job summary

Talent Connect Executive Search & HR Solutions is recruiting a Licensed Vocational Nurse (LVN) for a healthcare organization providing PACE services. The LVN will deliver compassionate, participant-centered care to older adults with complex needs under RN supervision, promoting health, independence, and quality of life.

Responsibilities include administering medications, documenting care, and collaborating with the care team.

Qualifications

• Current California LVN license in good standing.
• BLS certification required.
• Strong clinical observation and documentation skills.
• Ability to recognize and report changes in participant condition.
• Ability to collaborate with multidisciplinary healthcare team.
• Commitment to respectful, culturally responsive elder care.

Responsibilities

• Provide nursing care within the California LVN scope of practice per orders and care plans.
• Document vital signs and clinical observations; report changes to RN or provider.
• Administer medications and treatments as ordered and within competency.
• Perform wound care, specimen collection, and other nursing procedures.
• Support participant visits and prepare treatment areas; assist with daily clinic operations.
• Educate participants and caregivers on medications and chronic condition management.
• Collaborate with interdisciplinary team to communicate needs and support follow-up care.
• Maintain accurate documentation in the electronic health record.
• Follow infection prevention procedures and safe medication handling.
• Assist with emergency response per training and protocols.
