CarePartners Brevard, NC, is seeking a Licensed Practical Nurse (LPN/LVN) for our PACE Center. The role supports the Day Center, triaging participants, administering medications, and coordinating care with a multidisciplinary team.

Under the Day Center Supervisor (RN), you will provide patient education, help with order entry for labs and procedures, and ensure quality, compassionate care while supporting the practice's goals.

Qualifications

• License as LPN/LVN is required.
• Basic Cardiac Life Support (BCLS) within 15 days of start date.
• First Aid Basic certification within 30 days of start date.

Responsabilités

• Triage participants and provide nursing care for comfort and wellbeing.
• Administer medications per MAR and standing orders; assist with labs and procedures.
• Perform wound care, dressing changes and coordinate with providers and family.
• Document care delivery and patient condition using Nursing Process.
