Job summary

CarePartners Health Services in North Carolina seeks a Licensed Practical Nurse (LPN/LVN) for its PACE program at the Brevard center. You will provide nursing care, triage participants, administer medications per MAR, and support the Day Center Supervisor with procedures and labs.

You will document patient conditions, coordinate with team RNs, and educate patients on medications and care plans, ensuring compassionate, evidence-based support for the elderly.

Qualifications

LPN/LVN license required and in good standing.
Associate degree or vocational school graduate.
Basic Life Support and First Aid certifications due within specified days.

Responsibilities

Triages participants and administers medications per MAR and standing orders.
Assists provider and supervisor with labs, procedures, and visits.
Changes dressings and provides wound care as needed.
Documents care delivery and patient condition using the Nursing Process.
Coordinates care with RN team and prioritizes urgent treatments.
Provides patient education on pharmaceuticals and care plans.
