Job summary

CarePartners Health Services is seeking a Licensed Practical Nurse for our PACE Center in Brevard, NC. You will join a strong team to provide nursing care for participants, triage, administer meds per MAR, and support the Day Center operations.

You will collaborate with RN peers and providers, documenting patient care and educating patients on medications while supporting our care coordination efforts.

Qualifications

• LPN/LVN license required.
• BCLS must be obtained within 15 days of start date.
• First Aid Basics must be obtained within 30 days of start date.

Responsibilities

• Triage participants and administer medications per MAR and standing orders.
• Assist with ordering labs, procedures, and visits; support provider and Day Center Supervisor.
• Change dressings and provide wound care; coordinate with care team.
• Document care delivery and patient status; communicate with families.
