Job summary

TALENT CONNECT EXECUTIVE SEARCH & HR SOLUTIONS is recruiting a Licensed Vocational Nurse (LVN) for an onsite PACE program in California. The LVN will provide compassionate, participant-centered nursing care to older adults with complex medical needs, reporting to the RN or authorized provider.

Responsibilities

• Provide nursing care within the California LVN scope of practice and follow care plans.
• Document vital signs and report abnormal findings promptly to RN or provider.
• Administer medications and treatments as ordered and monitor responses.
• Perform wound care, specimen collection and other nursing procedures as assigned.
• Support visits, prepare treatment areas, and assist daily clinic operations.
• Educate participants and caregivers on medications and chronic condition management.
• Collaborate with the interprofessional team to coordinate follow-up care.
• Maintain accurate, timely documentation in the EHR.
• Follow infection control procedures and safe medication handling.
• Assist with emergency response per training and protocols.

Talent Connect Executive Search & HR Solutions is recruiting aLicensed Vocational Nurse (LVN) on behalf of a healthcare organization providing Programs of All-Inclusive Care for the Elderly (PACE).

The LVN will provide compassionate, participant-centered nursing care to older adults with complex medical needs. Working under the direction of a Registered Nurse or authorized provider, this professional will support coordinated care that helps participants maintain their health, independence, and quality of life in their communities.

Key Responsibilities

• Provide nursing care within the California LVN scope of practice, following provider orders, established care plans, and organizational policies.
• Collect and document vital signs, symptoms, and clinical observations; promptly report abnormal findings and changes in participant condition to the RN or provider.
• Administer medications and treatments as ordered, within licensure and demonstrated competency, and monitor and report participant responses.
• Perform assigned wound care, specimen collection, and other authorized nursing procedures.
• Assist with participant visits, prepare treatment areas, and support daily clinic operations.
• Reinforce participant and caregiver education on medications, chronic condition management, and established care instructions.
• Collaborate with the interdisciplinary team to communicate participant needs and support follow-up care.
• Maintain accurate, timely documentation in the electronic health record.
• Follow infection prevention procedures and support safe medication handling, equipment use, and supply management.
• Assist with emergency response according to training and organizational protocols.

Required Qualifications

• Current California Licensed Vocational Nurse license in good standing.
• Graduation from an approved vocational nursing program.
• Current Basic Life Support (BLS) certification.
• Strong clinical observation, communication, and documentation skills.
• Ability to recognize and promptly report changes in participant condition.
• Ability to work collaboratively with a multidisciplinary healthcare team.
• Commitment to respectful, culturally responsive care for older adults.

Preferred Qualifications

• Experience in PACE, geriatric care, ambulatory care, home health, or long-term care.
• Experience supporting individuals with multiple chronic conditions.
• Bilingual English/Spanish skills.
• Experience with electronic health records.
• Relevant procedure-specific training or certifications.

Work Environment and Schedule

This position supports an onsite, community-based PACE program. Responsibilities may include care in the PACE center, participants’ homes, or other assigned settings based on program needs.
