Job summary

Trilogy Health Services, LLC in Mount Washington, KY is seeking a Licensed Practical Nurse (LPN) to deliver direct, high-quality nursing care and supervise nursing assistants on the unit.

You will observe residents daily, respond to changes in condition, administer medications, and complete documentation, charting, and reports with a focus on safety, compassion, and collaborative teamwork within a supportive Thrive benefits culture.

Qualifications

• Must hold a current, valid KY LPN license.
• CPR certification required.
• Provide direct nursing care and supervise CNAs on unit.

Responsibilities

• Visit residents daily to assess status and notify charge nurse or physician if changes occur.
• Identify problems and initiate lifesaving measures in absence of physician.
• Provide holistic care within LPN scope and supervise nursing assistants.
• Monitor medications, supplies, and equipment and ensure adequate inventory.
• Complete medical forms, reports, evaluations, studies, and charting as needed.
• Report occupational exposures per facility policies.
