Job summary

Trilogy Health Services, LLC in Lebanon, IN is seeking a Licensed Practical Nurse (LPN) to provide direct, quality clinical care and supervise nursing assistants on the unit. You will visit residents daily, monitor their physical and emotional status, respond to changes, perform administrative tasks, and ensure medication and supply readiness while maintaining CPR-certified practice.

Qualifications

• Must hold a current, valid LPN license in good standing.
• Current CPR certification required.

Responsibilities

• Visit residents daily to assess status and notify charge nurse/physician/Director of Nursing if changes occur.
• Identify problems and emergencies and initiate lifesaving measures when physician is unavailable.
• Provide holistic care for residents within LPN scope.
• Assist in monitoring medications, supplies, and equipment to ensure availability.
• Complete medical forms, reports, evaluations, and charting as needed.
• Report occupational exposures per facility policies.
