Qualifications

• Provides basic healthcare services per individual plans.
• Performs delegated tasks such as blood pressure monitoring and weight checks.
• Maintains accurate and complete documentation per regulations.
• Keeps supplies and equipment organized and restocked.
• Collaborates with Direct Support Professionals to meet individuals' needs.

Responsibilities

• Assists in providing basic healthcare services according to the individual’s plan ensuring each individual’s optimal health and wellbeing.
• Performs tasks as delegated, such as blood pressure monitoring, weights, and other assigned tasks.
• Ensures all documentation is accurate and complete according to regulations.
• Responsible for the organization and cleanliness of supplies and equipment, ensuring restocking of inventory.
• Collaborates with Direct Support Professionals to ensure that the needs of the individuals are being met.
• Communicates immediately any changes in medical or behavioral status to supervisor.
• Performs all other duties as assigned.
