Job summary
Sunrise Community, Inc. is seeking a Licensed Practical Nurse to provide basic healthcare services to individuals with intellectual and developmental disabilities in Augusta, GA.
The role involves patient monitoring, documentation, and coordination with Direct Support Professionals to support individual health plans. Qualified candidates hold a Practical Nurse license, have a valid driver’s license with endorsements and at least 3 years of driving experience, and possess basic computer skills.
Qualifications
- Must pass pre-employment background and drug screenings.
- Must possess a valid Driver’s License with endorsements and have at least 3 years driving experience.
- Must possess basic computer skills.
- Must possess current and valid Practical Nurse license.
- Experience working with people with intellectual/developmental disabilities preferred.
Responsibilities
- Assists in providing basic healthcare services per individual plan to ensure optimal health.
- Performs tasks delegated (e.g., blood pressure monitoring, weights) and other assigned tasks.
- Ensures all documentation is accurate and complete per regulations.
- Organizes and restocks supplies and equipment to ensure readiness.
- Collaborates with Direct Support Professionals to meet individuals' needs.
- Communicates any changes in medical/behavioral status to supervisor immediately.
- Performs other duties as assigned.
