Job summary

Sunrise Community, Inc. is seeking a licensed practical nurse to provide basic healthcare services for individuals with intellectual and developmental disabilities. You will work within individual plans, monitor vital signs, and ensure accurate documentation.

Qualifications

• Must pass pre-employment background and drug screenings.
• Must hold valid Practical Nurse license.
• Must have a valid driver's license and at least 3 years driving experience.
• Experience working with individuals with intellectual/developmental disabilities preferred.

Responsibilities

• Assists in providing basic healthcare services according to the individual’s plan ensuring each individual’s optimal health and wellbeing.
• Performs tasks as delegated, such as blood pressure monitoring, weights, and other assigned tasks.
• Ensures all documentation is accurate and complete according to regulations.
• Responsible for the organization and cleanliness of supplies and equipment, ensuring restocking of inventory.
• Collaborates with Direct Support Professionals to ensure that the needs of the individuals are being met.
• Communicates immediately any changes in medical or behavioural status to supervisor.
• Performs all other duties as assigned.
