Job summary

PruittHealth Hospice is hiring a Licensed Practical Nurse (LPN) Palliative Care Coordinator to support patient care in home or facility settings. The role partners with the nurse practitioner and medical director to implement goals of care and ensure timely visits.

This position emphasizes care coordination, accurate documentation, and collaboration across interdisciplinary teams to enhance patient outcomes and comply with policies.

Qualifications

• Current, active LPN license in the state of practice.
• Three years minimum nursing experience.
• Hospice, palliative, or home health care experience preferred.

Responsibilities

• Take and process palliative care referrals.
• Support the Palliative Care Nurse Practitioner and Medical Director.
• Schedule visits to support goals of care as ordered by MD.
• Coordinate and communicate patient care goals between providers.
