Job summary

Liberty Hospice is seeking a dedicated Office Nurse (LPN) to support hospice patients and coordinate transfers from home health services, SNF, or hospital settings to palliative care. The role is full-time and based in our hospice office, requiring travel as needed to ensure uninterrupted patient care.

The nurse will provide patient care, assist in medication management, and support documentation and branch office tasks under policy and regulatory guidelines.

Qualifications

• Requires current LPN licensure within state of branch location and two years of clinical nursing experience, preferably one year in Home health/Hospice setting.
• Must be able to independently organize work and provide effective health care services to hospice patients; ensure appropriate care is documented thoroughly.
• Must accept travel and working conditions as assigned.
• CPR certification and minimum 12 hours of continuing education in a 12-month period.

Responsibilities

• Assist in coordinating transfer of patients from home health to Palliative or Hospice to ensure uninterrupted medical care.
• Coordinate transfers from SNF or hospital to hospice services to maintain continuous care.
• Provide patient care using a variety of skills and service coordination.
• Complete documentation as required by policy and regulation.
• Assist in day-to-day branch office tasks to ensure timely patient care documentation.
• Assist in management of patient care supplies.
• Assist in Medicare Part D medications management.
• Assist the Hospice RN with medication reconciliation and pharmacist recommendations.
