Maxim Healthcare in La Junta, CO is hiring a Licensed Practical Nurse (LPN) to work with adult patients in their own homes. Full-time, flexible part-time and PRN are available for La Junta, Ordway, Las Animas and Lamar areas.

• Provide skilled nursing care/services per prescribed orders on each assignment.
• Assess signs and symptoms indicating physiologic and psychosocial changes in the patient.
• Collect, analyze, and document data and nursing diagnoses.

Responsibilities:

• Utilize the nursing process to assess, plan, implement, and evaluate patient care.

• Collects, analyzes, and interprets data and information from health care members and documents actual and/or potential nursing diagnoses.

Qualifications:

• Currently licensed as an LPN/LVN in the state in which the LPN will practice.

• One year prior professional nursing experience preferred
