Job summary

Maxim Healthcare in Omaha is seeking a Licensed Practical Nurse (LPN) to provide home health services to pediatric and/or adult patients. The role offers full-time or part-time day and overnight hours, with competitive pay and comprehensive benefits.

The position supports patient care in home settings, using the nursing process, administering medications, and coordinating plans of care with prescribers and supervisors. Prior LPN experience and current licensure are required.

Qualifications

• Current Licensed Practical Nurse (LPN) license for the practice state.
• Current Health Certificate as required by contract/state regulation.
• Current PPD or Chest X-Ray and a valid BLS card.

Responsibilities

• Utilizes the nursing process to plan, implement, and evaluate patient care in home health.
• Initiates and revises plans of care as patient status changes.
• Performs therapeutic, preventive, and rehabilitative nursing actions.
• Intervenes for changes in patient condition and communicates with physician or supervisor.
• Administers medications and documents effectiveness of interventions.
• Reviews care plans and coordinates education, discharge planning and care coordination.

Responsibilities:

• Utilizes the nursing process to plan, implement, and evaluate patient/client care and/or services provided to client(s) receiving contracted services, as required
• Initiates the plan of care when directed and requests from prescriber any necessary revisions as patient status and needs change
• Incorporates therapeutic, preventive, and rehabilitative nursing actions based on patient diagnosis and established goals
• Intervenes as applicable in response to changes in patient/client condition and reports such changes to appropriate physician or supervisor
• Administers medications and treatments as prescribed, as well as, monitors and documents effectiveness of intervention
• Reviews plan of care and program protocols and identifies areas for healthcare teaching/education, care coordination, goal setting and discharge planning needs, as applicable

Requirements:

• Current Licensed Practical Nurse (LPN)for the state in which the nurse practices.
• Current Health Certificate (per facility Licensed Practical Nursecontract or state regulation).
• Current PPD or Chest X‑Ray.
• Current BLS card.
• One year prior Licensed Practical Nurse(LPN) experience preferred.
