Job summary

Intersect Healthcare, Inc. is seeking a Licensed Practical Nurse to provide quality nursing care in a long-term care setting.

You will collaborate with the Director of Nursing and Unit Managers to implement care plans, manage medications, and ensure compliance with state and federal regulations. You will assess residents, document observations, schedule rounds, supervise CNAs, and participate in regulatory surveys.

Qualifications

• Must possess a current Michigan license as a licensed practical nurse.
• CPR certification required.
• Two years of professional nursing experience in the long-term care setting is preferred.

Responsibilities

• Assess resident conditions and develop Care Plans; document observations.
• Record observations in PCC and follow through on Care Plans.
• Round with physicians; monitor symptoms and reactions to treatments.
• Provide preventive, supportive, maintenance, and rehabilitative care.
• Administer medications as prescribed and monitor for contraindications.
• Coordinate supplies, equipment, and appointments for residents.
• Supervise CNAs and report incidents to the DON/Administrator.
• Complete weekly, periodic, and discharge summaries.
