Job summary

Bethesda Health Group is seeking a Licensed Practical Nurse to provide essential nursing care for residents. You will perform admissions assessments, monitor ongoing changes, and communicate with the interdisciplinary team and families.

Timely documentation and adherence to the care plan are required. Responsibilities include medication administration, IV care if certified, and using SBARR for physician communication.

Qualifications

• Graduate from an accredited nursing program resulting in LPN licensure.
• Current LPN license in the state of employment.
• CPR and AED certification required or obtainable within six months of hire.
• IV certification required or obtainable within six months of hire.
• Long-term care experience preferred.
• Ability to assess basic nursing care needs of geriatric residents.
• Excellent communication skills with residents, families and staff.
• Time management and organizational skills required.

Responsibilities

• Provide basic nursing care and assess residents on admission and on an ongoing basis.
• Document assessments accurately and timely; communicate changes to the IDT and residents/families.
• Implement the care plan and nursing interventions per plan, including orders and treatments.
• Administer medications and treatments safely; IV administration if certified.
• Use SBARR for physician communication and maintain composure during crises.
