Job summary

Nashville Center For Rehabilitation and Healing is seeking a Licensed Practical Nurse (LPN) for full-time, rotating 12-hour shifts (7-7) in our Skilled Unit and Long Term Care Unit. Immediate hire with sign-on bonus.

Responsibilities include medication administration, including IVs, taking vital signs, resident care and treatments, education for residents and families, and coordination with physicians and CNAs, with supervision of CNAs.

Qualifications

• Current Tennessee LPN license is required.
• Experience in SNF or long-term care preferred.
• Ability to work 12-hour shifts on days or nights.

Responsibilities

• Provide care for residents in Skilled/Long Term Care units.
• Administer medications, including IVs.
• Take and record vital signs.
• Monitor treatment outcomes and adjust care as needed.
• Educate residents and families.
• Communicate with physicians, NPs, and care teams.
• Supervise CNAs.
