Job summary

Nashville Center For Rehabilitation and Healing is seeking a licensed practical nurse (LPN) for full-time, rotating 12-hour shifts (7am-7pm or 7pm-7am) in Belle Meade, TN. This role involves direct patient care, medication administration, vital signs, and collaboration with a multidisciplinary team.

The successful candidate will hold a Tennessee LPN license, have strong communication skills, and be able to supervise CNAs while delivering compassionate care in a skilled nursing facility.

Qualifications

• Current Tennessee LPN license.
• Experience with long-term care is a plus.
• Ability to provide education to residents and families.

Responsibilities

• Provide care for residents in SNF and Long Term Care Unit.
• Administer medications, including IVs.
• Take and record vital signs.
• Provide resident care and treatments with monitoring of outcomes.
• Educate residents and families.
• Communicate with physicians, NPs, and other health care professionals.
• Supervise CNAs.
