CarePartners Health Services is seeking an LPN/LVN to join the PACE Center in Brevard, NC. The role involves triaging, administering medications, wound care, and collaboration with the care team to support aging adults in a comprehensive care program.

The nurse will document patient status, educate patients about medications, and coordinate with the Day Center Supervisor to ensure quality care and adherence to orders and policies.

Qualifications

• Basic Cardiac Life Support must be obtained within 15 days of employment start date.
• First Aid Basic must be obtained within 30 days of employment start date.
• LPN/LVN licensed practical or vocational nurse.
• Associate Degree, or Vocational School Graduate.

Responsabilités

• Triages participants; administers medications per MAR and standing orders.
• Assist provider & Day Center Supervisor with ordering labs, procedures, and visits.
• Changes dressings and provides wound care; serves as liaison with providers, participants, staff, and families.
• Documents care delivery, patient condition, reaction and progress; uses Nursing Process.
• Provides patient education, especially regarding pharmaceuticals and care goals.
