Job summary

CarePartners Health Services seeks an experienced Licensed Practical Nurse for our PACE Center in Brevard. Under supervision, you will deliver compassionate nursing care, triage participants, administer medications, and support providers with labs and treatments.

You will educate patients about medications, coordinate care with the Day Center team, and document patient status using established nursing processes. Experience in elder care is preferred.

Qualifications

• Basic life support and first aid certifications required.
• LPN/LVN license and an associate degree or vocational credential are required.

Responsibilities

• Triages participants and administers medications per MAR and standing orders.
• Assists providers with orders for labs and procedures; performs wound care and dressings.
• Educates patients on pharmaceuticals and coordinates care with the nursing team and families.
• Documents care delivery and patient status using standard nursing processes.
