Job summary

Harmony Senior Services in Nashville is seeking an LPN to provide high-quality nursing care for residents in an assisted living setting. You will coordinate physical, social, and emotional care per physician orders and resident choices.

You will perform medication management, continuous assessments, and ensure wellness and safety with meticulous documentation. Join a caring team dedicated to resident well-being and community involvement.

Qualifications

• Current LPN license to practice in the state where the community is located.
• Experience working with the senior population, preferably in assisted living or LTC.
• Must be a team player who works well with others and has experience supervising staff.
• Must possess critical thinking skills, ability to effectively communicate verbally and in writing.
• Demonstrated ability to manage time efficiently with strong organizational skills.
• Must be in good physical condition and be able to sit, stand, bend, kneel, walk, and lift up to 50 lbs.
• Must be willing to work an 8-16 hour flexible schedule to include weekends and holidays.
• Must attend all required trainings, CEU’s and staff meetings.
• Must be certified or willing to take and pass the certification class required by the state in which employed.

Responsibilities

• Execute medication management plan including periodic review and communication with physician.
• Continuously assess resident needs and update care / services as condition changes.
• Implement measures to ensure resident wellness and safety with proper documentation.
• Work in a team to meet resident needs.
• Respond to emergencies timely and calmly.
• Report incidents and changes in resident condition to Healthcare Director with documentation.
• Be active in all activities and community events.
• Assist with delivery of meals and monitor nutrition.
• Maintain high standards of record keeping and documentation.
• Establish positive working relationships with all departments.
• Identify risk factors and initiate interventions with proper documentation.
• Provide thorough shift reports and updates in Daily Log.
• Motivate and direct CNAs/RMAs.
• Attend required trainings and CEUs and staff meetings.
• Adhere to policies and procedures and safe body mechanics.
• Maintain attendance and be prepared for duties.
• Complete all other assigned duties.
