Job summary

Harmony Senior Services in Memphis, TN is seeking an experienced LPN to provide high-quality nursing care for residents in an assisted living setting. You will coordinate care, manage medications, monitor resident condition, and support a team of CNAs and RMAs to ensure safety and well-being.

The role requires strong clinical skills, effective communication, and the ability to work a flexible schedule including weekends and holidays in a residential community.

Qualifications

• Must hold a current LPN license to practice nursing in the state where the community is located.
• Experience with seniors in assisted living or LTC preferred.
• Team player with ability to supervise staff and work with others.
• Strong verbal and written communication, critical thinking, and time management.

Responsibilities

• Provide quality nursing care to residents in assisted living according to physician orders and resident choices.
• Coordinate physical, social, and emotional aspects of care.
• Execute medication management plan and communicate changes to the physician as needed.
• Assess resident needs and update care plans with changes in condition.
• Maintain accurate documentation and records with high standards.
• Respond to emergencies promptly and calmly.
• Assist with meals and monitor nutrition and dietary acceptance.
• Lead and motivate CNA/RMA staff and participate in staff trainings.
