Responsibilities

• Perform focused assessments and collect data to update the Plan of Care.
• Implement nursing interventions delegated by RN or physician.
• Document care provided accurately and report changes promptly to RN.
• Ensure safety and compliance with HIPAA, BON standards, and facility policies.
• Coordinate with interdisciplinary team to support resident well-being.
• Lead by example in patient-centered care and facility safety.
