Campbell Street is seeking a dedicated LPN to provide high-quality, direct routine care to residents under the supervision of an RN. You’ll participate in care planning, document care activities, and support the facility’s QAPI goals while upholding BON standards and Residents’ Rights.

Qualifications

• Completion of a professional nursing education program approved by the state BON.
• Current License as a Practical Nurse for the state.
• Current CPR certification.

Responsibilities

• Plans of Care: conducts focused resident assessments and informs RN.
• Assists in developing and implementing the Plan of Care.
• Delivers direct routine nursing care under RN supervision.
• Maintains resident dignity, privacy and autonomy.
