Harmony Senior Services is seeking a dedicated Licensed Practical Nurse to provide quality nursing care in our senior living communities. You will coordinate resident care with physicians, oversee medication plans, and monitor safety and nutrition under the physician's orders and resident preferences.

You will work closely with CNAs/RMAs and other departments, responding to emergencies and documenting changes. Join a values-driven team delivering compassionate care across all shifts.

Qualifications

• Current LPN license to practice nursing in the state where the community is located.
• Experience working with seniors, preferably in assisted living or LTC.
• Team player with experience supervising staff.
• Strong critical thinking and communication skills.
• Good time management and organizational abilities.
• Physically able to sit, stand, bend, lift up to 50 lbs.
• Willingness to work an 8-16 hour flexible schedule including weekends/holidays.
• Attend required trainings and CEUs.
• Willing to obtain state required certification.

Responsabilités

• Provide quality nursing care and coordinate resident care per physician orders and resident choices.
• Execute medication management plan with documentation review and communication to the physician.
• Continuously assess resident needs and update care plans as needed.
• Ensure resident wellness, safety, and proper documentation of measures.
• Collaborate with team to meet resident needs and respond to emergencies calmly.
• Report incidents/changes to Healthcare Director with proper documentation.
• Participate in activities and assist with meals, monitoring diets and nutrition.
• Maintain high standards of record-keeping and documentation.
• Establish positive relationships with all departments.
• Identify resident risk factors and document interventions.
• Provide thorough shift-change reports and updates in Daily Log.
• Motivate and direct CNAs and RMAs.
