Job summary

Harmony Senior Services is seeking an experienced Licensed Practical Nurse to provide quality nursing care for residents in a senior living community. The LPN will coordinate care per physician orders, manage medications, respond to emergencies, and document all changes in resident status.

The role requires community-based nursing experience, teamwork, strong communication, and the ability to work a flexible schedule including weekends and holidays.

Qualifications

• Current LPN license to practice nursing in the state where the community is located; license in good standing with the Board of Nursing
• Experience working with the senior population, preferably in assisted living or LTC
• Must be a team player who works well with others and has experience supervising staff
• Must possess critical thinking skills, ability to effectively communicate verbally and in writing
• Demonstrated ability to manage time efficiently with strong organizational skills
• Must be in good physical condition and be able to sit, stand, bend, kneel, walk, and lift up to 50 lbs
• Must be willing to work an 8-16 hour flexible schedule to include weekends and holidays
• Must attend all required trainings, CEU’s and staff meetings
• Must be certified or willing to take and pass the certification class required by the state in which employed

Responsibilities

• Coordinate physical, social, and emotional aspects of care per physician orders and resident choices
• Execute medication management plan with documentation review and communication to physician when changes are needed
• Respond to emergencies calmly and promptly; report incidents to Healthcare Director with documentation
• Maintain and update Daily Log; provide thorough status reports on shift changes
• Assist with meals in the dining room; monitor nutrition and dietary adherence
• Establish positive relationships with all departments and maintain quality record keeping
