Job summary

Discovery Village at Deerwood is seeking a Licensed Practical Nurse (LPN) to enrich the lives of residents by delivering compassionate nursing care in a community setting. You will support resident safety, dignity, and well-being while following individualized care plans.

As an LPN, you will perform hands-on nursing tasks, monitor health changes, coordinate services, and collaborate with interdisciplinary teams to promote optimal resident outcomes in a vibrant Jacksonville community.

Qualifications

• Current LPN license or RN per state regulations.
• Minimum of two years licensed nurse experience.
• Experience in assisted living, long-term care, or skilled nursing preferred.
• Strong clinical judgment and communication skills.
• Ability to work nights, weekends, and flexible schedules.

Responsibilities

• Provide nursing care per resident care plans, policies, and procedures.
• Monitor residents' physical and psychosocial status; note changes.
• Administer meds and treatments as prescribed and document.
• Coordinate with physicians, families, pharmacies, and other care providers.
• Support health promotion and wellness initiatives within the community.
• Ensure compliant clinical documentation.
• Assist with care planning, service plan updates, and evaluations.
• Collaborate with caregivers and leadership for high-quality care.
• Respond to emergencies with sound clinical judgment.
• Provide training and guidance for care staff.
• Ensure compliance with regulations and participate in QA activities.
