Job summary

Discovery Senior Living seeks an attentive Licensed Practical Nurse (LPN) to provide hands-on nursing care for residents at our Trinity, Florida community. You will follow individualized care plans, monitor health status, and collaborate with a multi-disciplinary team to ensure safety, dignity, and well-being for each resident.

The role supports health promotion, accurate documentation, and timely communication with physicians, families, and care providers.

Qualifications

• Must hold an active LPN license in the applicable state or be a Professional Registered Nurse as per state regulations.
• Minimum of two (2) years of licensed nursing experience required.
• Experience in assisted living, long-term care, or skilled nursing preferred.

Responsibilities

• Provide nursing care to residents according to individualized care plans.
• Conduct ongoing assessments and monitor changes in condition.
• Administer medications and treatments as prescribed and document accurately.
