Discovery Village Trinity is seeking a dedicated Licensed Practical Nurse (LPN) to provide compassionate nursing care to residents in a senior living community. You will follow individualized care plans, monitor health, and collaborate with an interdisciplinary team to promote safety, dignity, and well-being.

This full-time role requires nights and weekends in some shifts, strong clinical judgment, and proficiency with electronic records.

Qualifications

• Current LPN license; eligible to practice in Florida.
• Minimum 2 years of licensed nurse experience.
• Experience in assisted living, long-term care, or skilled nursing preferred.
• Strong clinical judgment, problem-solving, and decision-making skills.
• Effective organizational, time management, and communication skills.
• Ability to manage multiple priorities and delegate appropriately.
• Commitment to seniors and person-centered care.
• Ability to work nights, weekends, and flexible schedules.
• Proficiency with electronic documentation systems and basic computer applications.
• Able to respond to medical emergencies.
• Exposure to bloodborne pathogens and infectious disease.
• Ability to lift or assist residents as required by care plans.

Responsabilités

• Provide nursing care to residents per care plans, policies, and procedures.
• Conduct ongoing assessments and monitor for changes in condition.
• Administer medications and treatments as prescribed and document appropriately.
• Coordinate care with physicians, families, pharmacies, and other providers.
• Support health promotion and wellness initiatives within the community.
• Ensure accurate, timely, compliant clinical documentation.
• Assist with care planning, service plan updates, and evaluations.
• Collaborate with caregivers and leadership to ensure quality care.
• Respond to resident concerns, incidents, and emergencies with sound clinical judgment.
• Support training and guidance for staff as appropriate.
• Ensure adherence to federal, state, and local regulations.
• Participate in QA initiatives, audits, and compliance activities.
