Job summary

Trinity Health Senior Communities, St. Paul’s Assisted Living in Indiana, seeks a compassionate Licensed Practical Nurse (LPN) for a full-time Day shift to provide resident care and support a safe, comfortable living environment.

You will administer medications, monitor vitals, supervise nursing staff, maintain accurate records, and ensure compliance with state regulations while collaborating with the Director of Nursing to uphold high-quality care for residents.

Qualifications

• Current LPN license in Indiana required.
• Experience in long-term care or assisted living preferred.
• Knowledge of medication administration and state regulations.
• Strong attention to detail and accurate record-keeping.
• Compassionate, patient and empathetic demeanor with teamwork.

Responsibilities

• Direct resident care including medications, wound care, mobility assistance, and vital signs.
• Supervise nursing staff during shifts and ensure adherence to care plans and regulatory standards.
• Maintain accurate medical records and ensure compliance with federal, state, and local regulations.
• Collaborate with the Director of Nursing or Clinical Care Coordinator to uphold quality care standards.
• Participate in quality improvement initiatives to enhance resident care and facility operations.
