Sedgwick County Health Center is seeking a Licensed Practical Nurse (LPN) to provide resident-focused nursing care in a 32-bed intermediate facility. Responsibilities include med administration, documentation, and collaboration with CNAs, RNs, and physicians to maintain high-quality care.

Ideal candidates will have Colorado LPN licensure, BLS, and prior long-term care experience, with strong communication and teamwork in a rural setting.

Formación

• Licensed Practical Nurse with Colorado license or compact state eligibility.
• Current BLS certification.
• Experience in long-term care or geriatric settings preferred.
• Strong documentation, communication, and clinical judgment.

Responsabilidades

• Observe and document resident status accurately and report changes promptly.
• Administer medications and perform treatments per orders and policy.
• Complete nursing admissions with required forms and assessments.
• Support care planning with observations and interdisciplinary input.
• Maintain professional communication with residents, families, and healthcare team.
