Job summary

Sauk County Health Care Center is seeking a Licensed Practical Nurse to perform LPN care tasks and supervise CNAs. The role covers night shifts (10:30pm to 7:00am) and involves medication administration, resident comfort, care plan discussions, and rounds with physicians.

Strong preference for geriatric nursing experience. Licensing requirements must be met, and accommodations are available as needed.

Qualifications

• Required: Licensed Practical Nurse License.
• Preferred: Nursing experience with the geriatric population.
• Any combination of education and experience that provides equivalent knowledge and skills.

Responsibilities

• Prepare and administer prescribed medication to residents and monitor the response.
• Provide comfort to emotionally distressed residents and families.
• Train and supervise CNAs, discuss care plans and evaluations.
• Monitor resident condition and report changes to the RN.
• Record nursing tasks and activity in resident records and ECS.
• Accompany physician on rounds and transcribe orders.
• Consult with physical therapists to develop restorative programs.
• Order and stock pharmaceutical supplies and medication.
• Other duties as assigned.
