Big-Bend-Woods-1 in Missouri is seeking a dedicated Licensed Practical Nurse (LPN) / Licensed Vocational Nurse (LVN) for a full-time day shift to provide high-quality patient care in long-term care, hospitals, and skilled nursing environments.

The successful candidate will demonstrate strong clinical skills, proficiency with PCC EHR, and compassionate communication to support geriatrics and dementia care, while ensuring patient safety, confidentiality, and collaboration with interdisciplinary

Qualifications

Active LPN or LVN license per state board.
BLS and/or ACLS certification preferred.
Experience in long-term care, hospitals, hospice, or outpatient clinics.

Responsibilities

Conduct comprehensive patient assessments with vital signs and history in PCC.
Administer medications via various routes following prescribed protocols.
Perform catheterization, tube feeding management, sterile processing and wound suturing when needed.
Monitor patient conditions, triage deterioration requiring intervention.
Collaborate with interdisciplinary teams to develop and implement individualized care plans.
Ensure HIPAA compliance and accurate medical records handling.
Educate residents and families on health management, medications, and discharge planning.
Participate in infection control practices and safety protocols.
