Job summary

Solero Behavioral Transitions in Pickerington, Ohio seeks an LPN for the day shift (7am–7:30pm, 3x12s) with weekend rotation. The role involves nursing assessments, medication administration, and collaboration with an interdisciplinary team to support residents with mental health and co-occurring disorders.

Strong candidates hold an active Ohio LPN license, hospital or behavioral health experience is preferred, and proficiency with EMR systems is expected.

Licensed Practical Nurse (LPN) — Day Shift

How You Will Make a Difference

• Provide compassionate, patient-centered nursing care to individuals receiving behavioral health and/or residential treatment services.
• Complete nursing assessments, obtain vital signs, administer medications, and provide treatments in accordance with physician orders and facility policies.
• Monitor patients throughout the shift, recognize changes in condition, and promptly communicate concerns to the appropriate clinical team members.
• Collaborate closely with RNs, providers, behavioral health staff, therapists, and other members of the interdisciplinary team to support individualized care plans.
• Administer and document medications accurately and safely, including completing medication reconciliation and addressing medication-related concerns.
• Participate in treatment planning, daily clinical communication, and interdisciplinary rounds as appropriate.
• Respond to medical and behavioral health needs calmly and professionally, following established emergency and safety procedures.
• Complete accurate, timely documentation in the EMR, including nursing notes, medication administration records, assessments, and patient updates.
• Support admissions, transfers, and discharges by completing nursing requirements, reviewing orders, and communicating pertinent patient information.
• Provide patient education regarding medications, treatment plans, wellness, and ongoing care needs.
• Maintain a safe, therapeutic, and respectful environment for patients, families, and team members.
• Communicate effectively during shift changes and provide thorough handoffs to ensure continuity of care.

What We’re Looking For

• Current, active LPN license in the state of Ohio.
• Previous nursing experience preferred; behavioral health, psychiatric, residential, or substance-use treatment experience is a plus.
• Strong assessment, medication administration, documentation, and clinical communication skills.
• Proficiency with EMR systems and electronic clinical documentation.
• Ability to remain calm and make sound decisions in a fast-paced clinical environment.
• Compassionate, dependable, and team-oriented approach to patient care.
• Strong attention to detail and commitment to patient safety.Ability to communicate effectively with patients, families, providers, and interdisciplinary team members.
• High school diploma or GED required.
• Must be 21 years of age or older.
