Job summary

Solero Behavioral Transitions in Pickerington, OH, is seeking a dedicated Case Manager to provide case management, engage clients in recovery, and coordinate care with clinical staff. The role requires an active CDCA certification in Ohio and a high school diploma or GED. Newly qualified grads are welcome.

Shifts are Monday–Friday, 8am–4:30pm, in a structured outpatient setting. The position emphasizes documentation, advocacy, and connecting clients with housing, employment, and healthcare

• Active CDCA certification in Ohio is required.
• High School Diploma or GED is required.
• Must be 21 years or older.
• Newly qualified graduates welcome.

Responsibilities

• Provide case management services, including assessment, service planning, and ongoing support
• Utilize peer support principles to engage and motivate clients in recovery
• Assist clients in accessing community resources such as housing, employment, and healthcare
• Maintain accurate and timely documentation in compliance with agency and state guidelines
• Collaborate with clinical staff to coordinate care and support treatment goals
• Facilitate individual and group sessions as appropriate
• Advocate for clients and promote a recovery-oriented environment

Solero Behavioral Transitions in offers a safe, structured environment for adults managing substance use and mental health challenges. As a dual diagnosis outpatient program, we provide integrated treatment for individuals with primary substance use or mental health disorders.

Key Responsibilities

• Provide case management services, including assessment, service planning, and ongoing support
• Utilize peer support principles to engage and motivate clients in recovery
• Assist clients in accessing community resources such as housing, employment, and healthcare
