• Strong clinical assessment, communication, and organizational skills
• Proficiency with EHR and Microsoft Office
• Ability to work effectively in a multidisciplinary team
• Must be 21 years of age or older

Responsibilities

• Assess, monitor, and document patients' physical and mental health status
• Administer medications and treatments as prescribed
• Monitor medication effectiveness, side effects, and adverse reactions
• Assist with admissions, assessments, and discharge planning
• Collaborate with physicians, therapists, case managers, and others to develop treatment plans
• Maintain accurate documentation in the EHR
• Educate patients and families regarding medications and plans
• Respond to medical emergencies and provide nursing interventions
• Ensure compliance with policies and procedures
• Participate in quality improvement initiatives and maintain patient confidentiality
• Promote a safe, respectful treatment environment
• Perform other nursing duties as assigned

Solero Behavioral Transitions in Youngstown offers a safe, structured environment for adults managing substance use and mental health challenges. As a dual diagnosis outpatient program, we provide integrated treatment for individuals with primary substance use or mental health disorders. Our services include individual and group therapy, case management, medication management, and life skills training designed to support long-term recovery and successful community reintegration.

Position Summary

Solero is seeking a compassionate, skilled, and dedicated Registered Nurse (RN) to join our clinical team. The Registered Nurse plays a vital role in delivering high-quality, patient-centered nursing care by assessing patient needs, administering medications, monitoring treatment progress, educating patients, and collaborating with the interdisciplinary treatment team to promote recovery and overall well-being.

Key Responsibilities

• Assess, monitor, and document patients' physical and mental health status.
• Administer medications and treatments as prescribed by healthcare providers.
• Monitor patients for medication effectiveness, side effects, and adverse reactions.
• Assist with patient admissions, assessments, and discharge planning.
• Collaborate with physicians, therapists, case managers, and other healthcare professionals to develop and implement individualized treatment plans.
• Maintain accurate and timely documentation in the electronic health record (EHR).
• Educate patients and families regarding medications, treatment plans, and healthy lifestyle practices.
• Respond promptly to medical emergencies and provide appropriate nursing interventions.
• Ensure compliance with all federal, state, local, and organizational policies and procedures.
• Participate in quality improvement initiatives and maintain patient confidentiality.
• Promote a safe, respectful, and supportive treatment environment.
• Perform other nursing duties as assigned.

Qualifications

Required

• Current Registered Nurse (RN) license in the State of Ohio.
