Licensed Practical Nurse (LPN) - Night Shift
How You Will Make a Difference
- Provide compassionate, patient-centered nursing care to individuals receiving behavioral health and/or residential treatment services throughout the overnight shift.
- Complete nursing assessments, obtain vital signs, administer medications, and provide treatments in accordance with physician orders and facility policies.
- Monitor patients closely throughout the night, recognize changes in condition, and promptly communicate concerns to the appropriate clinical team members.
- Administer and document medications accurately and safely, including completing medication reconciliation and addressing medication-related concerns.
- Maintain a safe, secure, and therapeutic environment while providing appropriate overnight patient observation and support.
- Respond calmly and professionally to medical, behavioral health, and other urgent needs, following established emergency and safety procedures.
- Complete accurate, timely documentation in the EMR, including nursing notes, medication administration records, assessments, observations, and patient updates.
- Monitor patient sleep, behavior, safety, and changes in condition and communicate significant findings to the clinical team.
- Collaborate with on-call providers, RNs, behavioral health staff, and other team members to address patient needs and maintain continuity of care.
- Support after-hours admissions, transfers, and other urgent clinical needs as assigned.
- Provide patient education and support related to medications, treatment plans, wellness, and ongoing care needs.
- Maintain awareness of the unit environment and promptly address safety concerns, changes in patient behavior, or other issues requiring intervention.
- Prepare a thorough and accurate handoff for the day-shift team, including patient status, overnight events, medication concerns, pending orders, admissions, and follow-up needs.
- Maintain a professional, respectful, and compassionate approach with patients, families, and team members.
What We're Looking For
- Current, active LPN license in the state of Ohio.
- Previous nursing experience preferred; behavioral health, psychiatric, residential, or substance-use treatment experience is a plus.
- Strong assessment, medication administration, documentation, and clinical communication skills.
- Proficiency with EMR systems and electronic clinical documentation.
- Ability to work independently and remain alert, organized, and responsive throughout the overnight shift.
- Calm and professional approach when responding to medical or behavioral health concerns.
- Strong attention to detail and commitment to patient safety and quality care.
- Compassionate, dependable, and team-oriented approach to patient care.
- Ability to communicate effectively with patients, families, providers, and interdisciplinary team members.
- High school diploma or GED required.
- Must be 21 years of age or older.
