Job summary

Empath Health's Trustbridge Hospice seeks a dedicated LPN to provide Continuous Care visits across Palm Beach County, delivering compassionate home-based hospice support 7 a.m.–7:30 p.m. with weekend coverage on alternating weeks.

As an LPN, you will work under supervision to assess needs, develop comfort-focused plans, and educate families, ensuring dignified end-of-life care. BLS and FL license required; IV course a plus.

Qualifications

• Active Florida LPN license.
• Current Basic Life Support (BLS) certification.
• 30 hour IV course per Florida Board of Nursing is a plus.

Responsibilities

• Assess patient and family needs under RN/physician supervision.
• Coordinate and provide individualized care plans emphasizing comfort and dignity.
• Utilize monitoring, intervention, and teaching skills to enhance quality of life.
• Provide emotional support during overnight Continuous Care visits.
• Ensure exceptional, personalized service for each patient and family.

Job description

Trustbridge Hospice, a member of Empath Health, is seeking a caring and motivated Licensed Practical Nurse (LPN) to provide Continuous Care hospice visits throughout Palm Beach County. This role offers the opportunity to support patients and families during critical moments by delivering compassionate, high-quality care in the comfort of their homes on the following schedule: 7am - 7:30pm and every other weekend.

Since 1978, Trustbridge Hospice has been honored to serve families across Palm Beach and Broward counties with compassionate, community-based care. Formed from the legacy of Hospice by the Sea and Hospice of Palm Beach County, we continue to provide comfort, dignity, and peace to those facing advanced illness.

What You'll Do

As a Continuous Care LPN, you will play a vital role in supporting patients and families during some of the most critical moments in care:

• Work under the direction of the Registered Nurse and primary physician to assess patient and family needs
• Coordinate and provide individualized care plans that prioritize comfort and dignity
• Utilize monitoring, intervention, and teaching skills to enhance quality of life
• Provide emotional support and reassurance to patients and families during overnight care
• Ensure exceptional, personalized service and a positive experience for every patient and family

What You'll Need

• Current license to practice as a Licensed Practical Nurse in the State of Florida.
• Current Basic Life Support Certification.
• Successful completion of 30 hour IV Course meeting State of Florida Board of Nursing requirements is a plus.
• Adequate and reliable means of transportation with adequate insurance and valid Florida driver's license.
