Job summary

Trustbridge Hospice, a member of Empath Health, seeks a caring Licensed Practical Nurse (LPN) to provide Continuous Care hospice visits across Palm Beach County. The role supports patients and families at critical moments in homes, with a schedule from 7am to 7:30pm and every other weekend.

As a Continuous Care LPN, you’ll work under the RN and physician to assess needs, develop care plans prioritizing comfort, and use teaching skills to improve quality of life while providing overnight care and

Qualifications

• Current license to practice as an LPN in Florida.
• Current Basic Life Support Certification.
• 30-hour IV Course meeting Florida Board of Nursing requirements is a plus.
• Adequate and reliable transportation with valid Florida driver’s license.

• Coordinate and provide individualized care plans prioritizing comfort and dignity.

Job description

Trustbridge Hospice, a member of Empath Health, is seeking a caring and motivated Licensed Practical Nurse (LPN) to provide Continuous Care hospice visits throughout Palm Beach County. This role offers the opportunity to support patients and families during critical moments by delivering compassionate, high-quality care in the comfort of their homes on the following schedule: 7am - 7:30pm and every other weekend.

Since 1978, Trustbridge Hospice has been honored to serve families across Palm Beach and Broward counties with compassionate, community-based care. Formed from the legacy of Hospice by the Sea and Hospice of Palm Beach County, we continue to provide comfort, dignity, and peace to those facing advanced illness.

What you'll Do

As a Continuous Care LPN, you will play a vital role in supporting patients and families during some of the most critical moments in care.

• Work under the direction of the Registered Nurse and primary physician to assess patient and family needs
• Coordinate and provide individualized care plans that prioritize comfort and dignity
• Utilize monitoring, intervention, and teaching skills to enhance quality of life
• Provide emotional support and reassurance to patients and families during overnight care

What You'll Need

• Current license to practice as a Licensed Practical Nurse in the State of Florida.
• Current Basic Life Support Certification.
• Successful completion of 30 hour IV Course meeting State of Florida Board of Nursing requirements is a plus.
