Job summary

Trustbridge is seeking a compassionate Hospice Licensed Practical Nurse (LPN) to join our Central Palm Beach County team. In this on-site role, you'll work 8:30am–5:00pm with weekend availability, providing patient-centered care within a hospice setting and coordinating with RNs and physicians.

• Current license to practice as a Licensed Practical Nurse in Florida.
• Current Basic Life Support Certification.
• 30-hour IV Course completion is a plus.
• Adequate transportation and valid Florida driver’s license.

Responsibilities

• As a Hospice LPN, assist in assessing patient and family needs under RN direction.
• Coordinate and provide individualized care plans prioritizing comfort and dignity.
• Utilize monitoring and teaching skills to enhance quality of life.
• Provide emotional support to patients and families, including during overnight care.
• Ensure compassionate, personalized service and positive experiences for patients and families.

Empath Trustbridge Hospice is seeking a compassionate and dedicated Hospice Licensed Practical Nurse (LPN) to join our team and provide high-quality, patient-centered care to individuals receiving hospice care in the Central Palm Beach County area. You will work 8:30am - 5pm with weekend availability.

What you'll Do

As a Hospice LPN, you will play a vital role in supporting patients and families during some of the most critical moments in care.

• Work under the direction of the Registered Nurse and primary physician to assess patient and family needs
• Coordinate and provide individualized care plans that prioritize comfort and dignity
• Utilize monitoring, intervention, and teaching skills to enhance quality of life
• Provide emotional support and reassurance to patients and families during overnight care
• Ensure exceptional, personalized service and a positive experience for every patient and family

What You'll Need

• Current license to practice as a Licensed Practical Nurse in the State of Florida.
• Current Basic Life Support Certification.
• Successful completion of 30 hour IV Course meeting State of Florida Board of Nursing requirements is a plus.
• Adequate and reliable means of transportation with adequate insurance and valid Florida driver’s license.
