Job summary

Trustbridge Hospice, part of Empath Health, seeks a compassionate Hospice LPN to join our Boca Area team and provide high-quality, patient-centered care to individuals receiving hospice services.

Schedule: 8:30am–5pm with weekend availability. You will work under the direction of the RN and physician to assess needs, coordinate care plans prioritizing comfort, and deliver personalized support to patients and families in a community-based setting.

Qualifications

• Current license to practice as a Licensed Practical Nurse in Florida.
• Current Basic Life Support Certification.
• 30 hour IV Course completion is a plus.

Responsibilities

• Assess patient and family needs under RN/physician guidance.
• Coordinate and implement individualized care plans prioritizing comfort.
• Utilize monitoring, intervention and teaching to enhance quality of life.
• Provide emotional support during overnight care.
• Ensure exceptional, personalized service and a positive experience for each patient and family.

Trustbridge Hospice, a part of Empath Health, is seeking a compassionate and dedicated Hospice Licensed Practical Nurse (LPN) to join our team and provide high-quality, patient-centered care to individuals receiving hospice care in the Boca Area.

Schedule: 8:30am - 5pm with weekend availability.

What You'll Need

• Current license to practice as a Licensed Practical Nurse in the State of Florida.
• Current Basic Life Support Certification.
• Successful completion of 30 hour IV Course meeting State of Florida Board of Nursing requirements is a plus.
• Adequate and reliable means of transportation with adequate insurance and valid Florida driver’s license.
