Job summary

Empath Health in Venice, FL is seeking a Continuous Care LPN to provide bedside observations, emotional support, and prescribed care under physician orders and RN supervision. The role focuses on maximizing patient comfort and quality of life in times of crisis.

You will work three 12-hour night shifts per week (8:00 PM–8:00 AM) with alternating weekends, covering Manatee, Charlotte, DeSoto, and Sarasota counties, including mileage reimbursement.

Qualifications

• Graduate of an approved nursing program with Florida LPN license.
• Minimum one year experience in acute care; hospice preferred.

Responsibilities

• Provide continuous bedside clinical observations and prescribed care under RN supervision.
• Report significant changes in patient condition to RN case manager and physician orders to maximize patient comfort.
• Assist in overnight triage and care center coverage as needed when CC cases are pending.

Schedule:
1:1 Patient Ratio |Three 12-hour shifts per week (8:00 PM – 8:00 AM), with alternating weekends (specific days to be determined).

What You'll Do

Under physician’s orders and RN supervision, the Continuous Care (CC) Licensed Practical Nurse (LPN) is responsible for the observation of patient/family needs and providing supportive care and emotional support to the patient/family unit in times of crisis in order to maximize patient comfort and enhance quality of life.

Primary tasks, duties, and responsibilities:

1. Under the direction of the RN case manager, provides continuous bed-side clinical observations, prescribed care, physical, and emotional support to patients as directed in plan of care, reporting significant changes in patient condition and family needs as needed.

2. In collaboration with the RN, accepts and processes physician orders to maximize patient comfort.

3. Continuous Care LPNs also assist Triage and Care Centers overnight when Continuous Care cases are pending.

What You'll Need

• Applicants must be a graduate of an approved nursing program and own a current Florida LPN license.
• Minimum one year experience in an acute care setting, providing direct patient care; previous experience as a hospice LPN preferred.
