Job summary

Trilogy Health Services is seeking a Licensed Practical Nurse (LPN) to provide direct clinical care and supervise nursing assistants on the unit. The role focuses on resident care, timely assessment, and collaboration with the nursing team to support safe, quality outcomes.

Qualifications

• Must have and maintain a current, valid state LPN license
• Current, valid CPR certification required

Responsibilities

• Visit residents daily to assess status and report changes to the charge nurse or physician
• Identify problems and emergencies and initiate life-saving measures if needed
• Provide holistic care within LPN scope
• Monitor medication inventory and ensure supplies are on hand
• Complete medical forms, reports, evaluations, and charting as needed
• Report occupational exposures per facility policies
