Job summary

Trilogy Health Services is seeking an LPN to provide direct, quality clinical care and supervise nursing assistants on assigned units. The role focuses on monitoring residents, addressing changes in condition, and ensuring timely reporting to the charge nurse and physicians.

The position requires an active LPN license and CPR certification, with responsibilities spanning patient care, medication oversight, and administrative duties within a supportive, growth-focused long-term care environment.

Qualifications

• Must have and maintain a current, valid state LPN license.
• Current, valid CPR certification required.

Responsibilities

• Visit residents on assigned unit daily to observe and evaluate health status and notify nurse/physician when there is a change.
• Identify resident problems and emergencies and initiate lifesaving measures in absence of physician.
• Provide holistic care within LPN scope.
• Assist in monitoring medications, supplies, and equipment inventory to ensure availability.
• Perform administrative duties such as charting and reports as needed.
• Report occupational exposures to blood and hazardous materials per facility policy.
