Trilogy Health Services, LLC is seeking a Licensed Practical Nurse (LPN) to provide direct, quality clinical care and supervise nursing assistants for residents on their unit. The role emphasizes observation, timely reporting of changes, and adherence to scope of practice.

Key duties include daily resident visits, lifesaving measures in emergencies, medication/inventory oversight, and completing required documentation. Current LPN license and CPR certification are required for this position.

Qualifications

• Must hold a current, valid state LPN license.
• Current CPR certification required.

Responsibilities

• Visit residents on assigned unit daily to observe and report changes to the charge nurse or physician.
• Identify resident problems and emergencies and initiate lifesaving measures when needed.
• Provide holistic care within the LPN scope of practice.
• Monitor medications, medical supplies and equipment to ensure adequate stock.
• Complete medical forms, reports, evaluations and charting as necessary.
• Report occupational exposures per facility policies and procedures.
