Job summary

Citadel Healthcare, located in Freeport, IL, seeks a Licensed Practical Nurse (LPN) to join our caregiving team. The LPN provides licensed nursing care, administers medications, and supports resident assessments under physician orders.

Qualifications

• Excellent written and verbal communication in English.
• Ability to multi-task and remain calm under stress.
• Adheres to infection control standards and procedures.

Responsibilities

• Provides licensed nursing care to residents as ordered by physician and per regulations.
• Maintains clean and sanitary work areas and prepares meds and treatments as ordered.
• Directs and supervises CNA services to ensure compliant care.
• Assists with resident assessments, care plans, and documentation.
• Charts care and resident responses; completes admission/transfer/discharge forms.
