Citadel Healthcare is seeking a licensed practical nurse (LPN) to join the team. The LPN will be responsible for patient care under physician orders and regulatory standards, including med administration, documentation, and care coordination with the IDT.

The role requires strong communication in English, clinical competence, and the ability to multitask in a fast-paced environment. Prior long‑term care experience is preferred, with complete CPR certification and an active state LPN license.

Formación

• Requires active Licensed Practical Nurse license in the state.
• CPR certification is required.
• 1 year of experience preferred; long-term care experience preferred.

Responsabilidades

• Provide licensed nursing care per physician orders and standards.
• Handover: report to shift and receive reports.
• Maintain clean and sanitary areas including stations and rooms.
• Supervise CNA activities on assigned unit.
• Assess residents, review care plans, and document interventions.
• Administer medications and treatments as ordered.
• Chart nursing notes with resident responses to care.
• Complete admission, transfer, and discharge forms.
