Job summary

Citadel Healthcare's The Citadel at Saint Anne Place in Rockford, IL is seeking a Licensed Practical Nurse (LPN) to provide licensed nursing care to residents, administer medications, monitor vital signs, and collaborate with the care team to follow care plans. The LPN will document care, follow infection control standards, complete admissions and discharge paperwork, and support CNAs under supervision.

A current IL LPN license and CPR certification are required; 1 year of long-term care

Qualifications

• Current CPR Certification required.
• Possess a current license to practice in the State as a Licensed Practical Nurse.
• 1 year of experience preferred.
• Long-Term care experience preferred.

Responsibilities

• Provides licensed care to residents as ordered and per standards.
• Gives and receives nursing reports at shift change.
• Keeps areas clean and sanitary; maintains equipment and rooms.
• Supervises day-to-day CNA services on assigned unit.
• Performs assessments/observations and reviews care plans.
• Monitors vital signs and administers medications and IV therapies as allowed.
• Transcribes physician orders to MAR and charts care.
• Documents nurse notes reflecting care and resident responses.
• Completes admission, transfer and discharge forms.
• Ensures safety and addresses risks promptly.
• Reports and recommends shift activities; supports care plan changes.
